Caring for your loved one at home
The Minute Man Arc Adult Family Care Program (AFC) provides qualifying family members/caregivers with financial and clinical assistance to care for their loved ones at home.
What is Adult Family Care?
If you are caring for an older adult or a young adult with a disability who is unable to independently complete their Activities of Daily Living, our AFC program can help.
Adult Family Care (AFC) is a MassHealth funded program designed for those who are unable to live on their own, but want the peace of mind and freedom of living in a home environment.
AFC offers assistance to an approved family member, friend, or qualified caregiver who is providing in-home supervision of someone who is chronically ill or has a disability.
Eligibility
- MassHealth Eligible
- 16 years of age or older
- Unable to live alone due to a medical, physical, cognitive, or mental condition
- Must have a qualified caregiver living in the home*
Benefits
- Tax-free caregiver stipend up to $1,580/month
- Caregiver support and health coaching
- Home visits from an RN and Case Manager
- Coordinating social services including referrals to Adult Day Health programs and respite care.
